Form submission returns form with empty fields

We are on JoomDonation 3.2 with Joomla 2.5.11.

On the following page, when the form is submitted a donation is not recorded and the original form is returned with empty fields. There are not any Javascript errors during processing.

I have noticed others have the same issue so I want to see what I should check to fix this.

There might be a plugin on your site causing the redirect and make the form empty. To make it easier for us to find out the problem and get it sorted, please submit a support ticket sending us admin and FTp account of the site so that we can check and correct the problem for you !
