problem with donationForm

hello,

i just install joom donation , and when i go on the page where is the module ( whatever is campaign module or and other)
i got that problem : first when i clik on donate button , the page change but nothing no form
second, something appears on the page when i click but its only , for exemple with campaign module, the campaign module .

why do you think i dont have the form , and how can i have ..

I have the same problem. Is the jdform a menu item & if not how can I create a jdform that can be inserted into an article, like your demo write-up indicates?
