I viewed the output of html code on donation form and sure that you are not having the latest code on your server. Just send me administrator account , I will work fine .

OK, but I just re-downloaded and re-installed and it is still not working. I will email the admin account details. What is the times-tamp of the version you will be installing?

I just re-installed the donation form plugin . The latest code is on the server, you can login to your account, download it and test it again .
