Custom Field need

I need to create a custom field like this..


I've tried to make this happen countless times, but I cannot figure this out.

I had asked if this module could do this functionality before I bought the module, and I'm sure it can, but I have been able to do this so far.

Has anyone run into this?

Basically, you need to create two custom fields : One is dropdown field type and one is textbox field type . However, because each custom field is displayed in one line, so this will need a small customization to the extension . If you can give me FTP account, I will be happy to customize it for free for you .

I would definitely like this customization done, but I am still waiting on another reply for customization for the CSV report and PayPal reporting.

Please let me know when I can send you this FTP info and I will be glad to provide it to you. But, I first need to have the CSV report issue resolved before I can ask for this.
