Number of occurrences textebox

The number of occurrences textbox does not appear on the online donation page. I have configuration set to show the textbox and campaign page is set to enable recurring donation.

In addition when I select the next box located at the end of the online donation form it does not go to the next page or give the option to submit payment.

Any assistance would be greatly appreciated
