We use the UPS shipping method, then actually ship through FedEx. The rate quotes for UPS are higher than the rates we actually get when shipping FedEx so we just refund the difference to our customers through our online credit card terminal.
At least this way we are not losing money on any shipments, or requiring anyone to call a customer and tell them we undercharged for shipping and its going to be more....Customers hate that...So the UPS rates plugin works the best for us.
On the other hand, customers LOVE getting a phone call telling them UPS quoted them to high, and since we will ship FedEx we are issuing a refund.
That said, the best scenario is an actual FedEx plugin so that customers see lower shipping rates in the cart, which helps us compete with other companies more effectively due to the overall cost to the customer being lower than some of our other competitors due to lower shipping costs once cost of the products + shipping costs are factored in.

It isn't quite ideal, it's not accurate, and it only covers the US.
