Form alignment doesn't look very nice

This is what my form looks like:



I want the text field titles to be vertically aligned with the top of the text boxes and I want the text area boxes to be wider, much like the 'comments' box.

Also, I would like the radio options to be listed vertically if possible. I see in an earlier thread that it wasn't possible to do this a few months ago, but I was wondering if there is a work-around now.

The misaligned form boxes are at the end of the form. I'm using this template:

Have you been able to determine what might be the problem?
