Frontend submit not working properly

Hello,

I've created a frontend submit item menu using the default EB layout.

None of my categories are appearing in the dropdown or the Additional Caltegories multi-select. Any user should be able to submit an event and the categories are set with Public access.

I'm also unable to type in textarea fields.

Did I miss some steps / settings?

EDIT: I'm also wondering if it's possible to display a category filter on the levent list menu item?
