No States Listed in States popup

HI Folks,
There is nothing to select in the "States" special field. It says "Select State" but nothing appears when you click on the Popup. I Checked the "States" configuration page, and all the States are published. The "United States" country is published too. 

Update: They are appearing, but only in the billing information form. There are a couple other places in the same form that the States are not appearing.

Actually, you can only have one country-state field on the form. It is not possible to have multiple countries/states field on the form yet

So on the registration form one would typically have a "personal info" as well as "billing info". How would I collect the state for each area?

For the one of the two state field, please use Text field type instead

What should I use for field validation?

Maybe just set it to Required
