Payment for Combine Fee Fields Form

Is the code for the example form 'Payment for Combine Fee Fields Form' available for me to customize?

The forms I need to build require the ability to select from multiple, individually-priced offerings.  The 'price' to be calculated based on the options chosen.  When I was looking for a forms package I noted the 'Combine Fee Fields' example form.  I see now that there are no instructions for how to make my form do this.  So, Now: What do I need to do to create the custom fields that are highlighted on this example?  Is the code for 'Payment for Combine Fee Fields Form' available for me to customize?

The documentation at  eventbookingdoc.joomservices.com/basic-s...ds/custom-fee-fields references a 'radio list' control.  I do not see this type of control in the dropdown list of controls available on my Joomla! 4 installation.  I suppose this control has been renamed to 'Radio.'   Please confirm.
I am attempting to create a new field by choosing "Components" and then "Fields" from the Joomla! 4 sidebar menu.  I see no other way to create a new 'field.'
When I arrive at the Articles:Fields to create the new field the 'Fee' field is not displayed as is shown in the example.  Please help me to understand what I've done wrong.
I have attached some screen shots that may help to understand where I'm going wrong.
