Group registration issue - no field available to enter data

I recently installed event booking on a website, all was ok, single and group registration was working.

Probably my customer change some settings and now the group registration button is there but no field available to enter data :

From my quick check, there is some javascript conflict on the page causing the error, I'm unsure where it comes from yet

Could you please submit a support ticket sending us super admin account of your site and link to the page so that I can check it further?
