Screen looks odd

When you click on "register" from the timeline layout, it brings this screen up that looks nothing like the others. How do i get it to look like the rest of the component? Screenshot included

Look like there is some javascript conflict and it causes the issue. Please submit a support ticket sending us super admin account of the site and the link to the page

We are going to check to see why the issue happens and get it sorted
