Required fields aren't treated as required was created by Bogdan Michka

I have a problem: On my form required fields aren't treated as required -- the form submits even if I leave the fields empty.
Try it here: aapgmcs.org/field-conferences/2016/addit...tion-how-to-register (select Offline Payment).
How do I fix it?
