Any chance you are using the SH404SEF component? I've found that sometimes if I change a menu item and don't purge the SEF URLS and let it rebuild them similar things will happen.

I'm assuming that all other URLs on your site resolve properly, but there could also be a problem between Joomla's SEF URLs and an incorrectly configured .htaccess file. If you have SEF URL enable in Joomla's main Configuration, make sure that you have copied the proper lines into your .htaccess files (assuming you're on a Apache web server) from Joomla's htaccess.txt file located in the root directory of your Joomla install.

The htaccess is Joomla's unmodified default, so that's not the problem.
