Location and Show Billing field last name is lost

On some events the settings made for location and show last name in billing setting is lost sometimes (dont know when it happens).
The result is that the list of registrants is missing last name.

The default setting for new events is show last name.

Any Ideas?

I also have problem with custom fields in the front end cant use them anymore. But thats another story.

Sorry but I have no idea (and to be honest, I don't know why it can happen and no one had problem like that before).

So could you please check it carefully and explain step by step how I can see the issue and fix it !
