Group billing is not working for event registration. I have enabled group registration for an event. But when redirected to Paypal they are only charge for the 1 person. I have custom fee field for registrants to select events to pay for. below is link for the event. I am including screen shots of the event config and the Custom fee fields. I can provide a test user to use for testing if you need.
