I am looking for a way to put multiple "agree to terms" check off boxs and text on my registration page with alot of text in the "terms" above it. For example, I would like to put these terms underneath the registration questions:

HAZING In addition to being in violation of General Laws, hazing is a most flagrant example of lack of respect for both self and others. General Law Chapter 269, section 17-19 was enacted in 1988. The law requires that secondary schools provide all school groups (e.g. athletic teams) a copy of the law. The Hazing Law is quoted below. Hazing in any form is unacceptable.
With an "I agree" Button or check box below it and text box to type in a name as a signatory.
The regular pop-up agree to terms wont work because I have several different sections of terms that I need to have acknowledged. Any suggestions would be appreciated.
