Confirmation page issues

I am having a few issues with my confirmation page, I saw others in the forum having the firefox issue but didn't see any resolution.
On my registration confirmation page I am having the following issues:
1) The "Total amount" field shows "[TOTAL_AMOUNT]" instead of an actual dollar amount.
2) There is a field that displays "EB_AMOUNT_DUE" instead of a human readable description and I don't see anywhere in the "Translations" section to change this.
3) When I click on the "Terms and Conditions" link using Firefox it opens a small, white light box window but doesn't actually display my "Terms and Conditions" article. Other times the whole page "grays out" but no box pops up. At this point you can't do anything else on the page. I've tried this on multiple computers with Firefox (3.6.2 and 9.0). The article displays properly in Safari and Chrome.
Any thoughts?
