I just setup the new waitlist feature and have 2 issues:
1. The waitlist form only asks for First name. We need last name too.
2. When I click on registration button, I only get a blank webpage.

Hi
=> The fields on Waiting List form is configurable . To show more fields, go to Event Booking -> Configuration, looks at Waiting List Fields, you will be able to choose the fields you want to show/hide, mark fields as require/not require .
