Search Module Layout Changes

We are hoping to customize the search module layout.
1. First field is a generic search field. Remove.
2. Move the Location field to the top.
3. Leave the From Date field.
4. Leave the To Date field.
5. Change the name above field "Category" field to "Camp Type".

You would need to modify code of the module to change the layout like that. I don't know exactly the layout you are using for search module, but you would have to change layout in one of the two files below:

- modules/mod_eb_search/tmpl/default.php
- modules/mod_eb_search/tmpl/horizontal.php

One the modification is done, you should move the modified file to template override templates/template_name/html/mod_eb_search so that the change won't be lost when you update to future releases of the extension

1. Your instructions worked like a charm.

2. The main configurations provide Theme > Misc > Show Search Bar. I have it on Yes, however I don't see it appearing on front end. I am making an assumption it would appear at top of our events. 
Should it be visible without using a module or is a module necessary for this feature? See screenshot.
