how to add instructions text box to the product

we need to small text box to the product so after the customer choose the kind of pizza then he choose the size then he adds the toppings, we need an instruction box so the customer will instruct based on these, for example some customers as to the topping half onions on the pizza and the other half is black olive, also we need another instruction text box at the check out so customer instructs the delivery driver if need it, please help

You can create an new option with type Textbox, then assign it to the product. So when shoppers add the product to the cart, then must enter the text first.
