Hello Martin,

Currently, there is no option to add more fields to the quote form. If you want to have that, then you must modify to the extension.

You can check the file components\com_eshop\themes\default\views\quote\default.php to modify it as the way that you need.

You should overwrite it from the template to avoid losing when update EShop in the future. Please read about it in the documentation at: eshopdocs.joomservices.com/developer-guide/customize-layouts
