Do the addresses on order such as payment address and shipping address include the country name now?

Hello David,

Please go to EShop -> System -> Configuration -> Layout tab, at Shipping Address Format and Payment Address Format, please add [SHIPPING_COUNTRY_NAME] and [PAYMENT_COUNTRY_NAME] there, then the country name will appear on the address of order, email, etc.

Also, make sure that you are using latest version 1.3.9 of EShop.
