Issue With "Add" Button was created by Jordan

Has anyone else had a problem with the "add" button in customer groups, discount, special, and images? The option menu will not allow me to select an option as well.
I am getting a Reference Error that says addProductAttribute is not defined.
This problem started out of nowhere this weekend, and I was curious if anyone had a solution.

The issue came from wrong name of customer groups. I have already explained and fixed that for you via ticket system.
