When I go through the checkout - the shipping cost is not showing up.  Can you please take a look and let me know what I did wrong?

There are 2 different methods when going through the checkout - PayPal and Cash at Door -- shipping needs to be applied to both....

You need to assign All Zone to Flat Shipping plugin. I just did that for you and it works now.
