Thumbnail uploads don't seem to work

Hi, 

We're 'on v3.0 and we're trying to add a thumbnail for a new document and it doesn't work. The person that has been doing this has done these document uploads with thumbnails for years - last month it worked but we were on the prior latest version of EDocman.

What do we need to do to get this to work?
