Download button configuration

Feeling a little silly asking this but... What configuration button do I change to get the download button to allow the popup to be active? When we click onto the download button the 'Your Name & Your Email' popup box shows. However everything is grayed out and when you click onto the popup box to enter your data the popup box goes away (screen shot attached). I quickly looked through the forum posts and adjusted every 'Download Configuration' button but so far, no luck getting this popup box to be active. Thanking you in advance for your time and assistance and sorry if I'm not seeing the obvious adjustment!

I have the same problem ! I use the template T3_bs3_blank.
In addition, the forms are displayed incorrectly, regardless of whether I use Bootstrab 2 or 3?

Could you please submit ticket into category: Edocman and provide your site url with super admin account. I will check and help you to solve the issue
