I get 400 downloads per day and there is no way we are actually getting that many real users downloading documents. Would bots hitting the pages cause this or is there something else that can cause it? and is there a way to prevent it?

In this case, i think you can turn on Configure option to Collect Download information. By that way, user should enter Name and Email before they can download documents.
