Frontend upload problem

*Problems on the frontend Upload.*
Problem #1
I upload a new file over Frontend Upload function.
I found out the file name has been missed on detail view page.
Problem #2
Login and access to edit document frontend page. The user clicked on "Cancel" button, but that document still has been locked. Other login user unable to access this document and edit it. The document has to unlocked by Administrator from joomla backend.
Anyone know how to fix them?
