Option for purchasing the file in hard copy

I am selling webinars and would love to have an option to let people purchase a DVD instead of the download (they are between a 100 to 150 MB wmv or zip file - but that is a very large download for some). If they choose to have it in hard copy format, I would want to add shipping (and tax) options. Right now I ask them to choose the offline purchasing method and include the shipping.

In my case I want to add the following flat rate shipping & handling options: $3.99 regular shipping for US and Canada, $6.99 for priority mail, $14.99 for International mail.

Tax would be dependent on where the person lived - in Washington, you have to charge tax to Washington residents. Although I would be willing to not have a tax option if I could have the shipping option.
