Shipping plugin issues

I have 3 problems:

1) USPS plugin is not showing all the options I choose and ALWAYS selects Large envelope (however, it does not display on front end.)

2) On the front end there are some HTML tags showing and the 2 options that do show are messed up.

3) Bought the free shipping pro (for 20 bucks there could be a doc) and can't seem to figure out how to apply it to purchases totaling $75 or more.
