Custom Fields Not Appearing

I tried sending a support request - but no response, so I'll try here:

When I added two custom fields in JoomDonation, I expected to be able to see them on the donation form. Unfortunately, they do not appear and there does not seem to be any place to turn them on other than the custom field tab. Both fields are enabled.

I am sorry for lately response. It is new year holiday in our country and we don't work much these days (we are going to come back to normal working schedule from Monday next week).

Please submit a support ticket sending me administrator and FTP account of your site and I will try to check the problem for you
