Field arrangement, and documentation

Hi,
First question:
is there any documentation/tutorials or videos except for the very basic one here ?

Second:

I need to create a form with the fields displayed in this order:

Drop down Field: (product)
textfield: (amount)
textfield: (Name)
textfield: (Surname)
email field: (Email)
etc.
etc.

The amount you need to pay is £x.xx

---

I deactivated all the standard fields from "Forms management" but there is no obvious way to rearrange the "Name" and "email" fields.

How do i do this?
