Default form fields not showing frontend

I seem to be getting a notice message of sorts when I install the component as well as having only the first name and email address fields showing in the front-end.

The notice message is as follows:

Notice: Trying to get property of non-object in /home/craftylion/www/client/components/com_pmform/views/form/view.html.php on line 176

The notice is repeated at least 6 times for 6 different lines within the view.html.php document. What exactly does that mean and how do I fix it?
