1. How do I insert a form in an article?
2. I would like to have one form but with the option of two different payment amounts the user can choose from. Is this possible?

=> - You need to install Payment Form plugin (the file pmform.zip in the package), then publish this plugin .
- Within article , using the syntax {pmform FORM_ID} . For example, If you want to display the form with ID = 1, entering this {pmform 1} .

1. is now sorted thanks - explanation fine.
2. sorry - this isn't clear to me. I would like to have the two different amounts (not methods) on the same form for the user to choose from.
and another..
3. When the form is redirected to PayPal, the PayPal description is blank ... it just says "Payment for" How can we have the form title in this area?

Sorry, I misunderstood you before . Here is the solution :
Regarding the #2 , you can use custom fee field feature of the extension. Please look at the attached image and see my explain below, you will understand :
- The title of the field is Member Type .
- Field Type is Radio
- In the values field, enter Silver, Gold (each item in one line)
- In the default values , enter Silver
- In the fee field , Choose Yes[/]b .
- In the fee values field , Enter 20,30 (each item in one line)
Then in the form, users can choose between Silver Member or Gold Member . If they choose Silve Member, they will have to pay 20$ . If they choose Gold member , they will have to pay 30$ .
Hope my sample is easy to understand .
Regarding #3, go to translation function of the extension, Look for language Item PF_PAYMENT_FOR and change it to Payment for [FORM_TITLE] (the item [FORM_TITLE] will be replaced by title of the form) .

ahhh...thanks...I sort this one in the next day or two. However, I can't see the attached image..sorry..

I've looked and looked and checked but still the PayPal window shows Payment for (then blank)
Item PF_PAYMENT_FOR is already set as Payment for [FORM_TITLE]
What should I do now do you think?

This should definitely be explained in the documentation, it is essential.
Unfortunately it does not work with me. When I fill {pmform 1} in an article, it appears just like that on the website - without any form.
Any idea what could be the reason and how I can change that?

I am sorry for lack of documents . I am working on new version at the moment . Once It completed, I will prepare guide document (should be ready by tomorrow) .
Regarding the issue with payment form plugin, could you submit a support ticket contain FTP account and Joomla administrator account of your site so that I can check it for you ?

I have solved the problem!
However a new one came up. All fields are displayed empty, so that the visitor can fill them out - except one, the email field, here I find a warning that javascript is disabled, which it is not. This seems to be a Firefox issue, since IE and Opera show the site fine.

Could you send me the site URL ? Maybe something with the email cloaking plugin . So the solution is :
In the Plugin Manager, I move Payment Form Plugin after Content - Email cloaking . After that , it will work . Could you try to do it and let me know the result ?
