This is the first option for the user to choose<br />
This is the second option for the user to choose<br />
This is the third option for the user to choose
The problem is that the items that have a <br /> at the end, when I get to paypal, the amount is $0 instead of the amount assigned to each item. The last item on the list works fine.
So how do I get my list to look like the attached and have the amount assigned to each item sent to paypal?

As replied on support ticket, I modified the code in the component alitle to make it display like you want. It all works fine now (tested it) . Please go to your site to check it !

I tested the form and it works perfect with paypal! I hope this can be included in the next version - in case I need to use it again I don't have to bother you.
Just one more question.
When I go to paypal and I click the browser back button - to go back to the site, then on the form's back button - to go to the original page that I was on, why does it not take me back to the original page?
What I mean is say my page is called www.mywebsite.com/make-a-payment.html when I click the back button it takes me to www.mywebsite.com/index.php .
I think it would be better that it takes the user back to the original page in case the user wants to read the instructions again. Because when they are on www.mywebsite.com/index.php all they see is the form that is embeded into the article. And, once they are on that page, there is no option to go to the original page, because there is only a "Next" button.

I, too, am interested in being able to do a custom layout for the form. That would include the ability to control the order, size of boxes, position of radio buttons (horizontal, vertical, indented, etc.), additional text inserted between form items, even font colors of individual items. Yes, that's a tall order; obviously not something we could expect you to do for us.
I'm comfortable messing with the html code to suit my needs. I just need to know if there's a way to access and set the html code for the form. (Another form component I use uses a "Smarty" template that generates the standard layout, but also allows us to make changes with Smarty tags and/or html tags. Unfortunately, it doesn't have a payment component with it.)
I've been using JoomDonation and it works great, but not being able to customize the layout is a big concern for me.

Infact, Payment Form allows you to make custom layout (if you know HTML code + alitle PHP), however, this feature is still "hidden" (I haven't had time to write a tutorial document yet) . Will try to spend sometime next week to write it up !

I don't see the "size attribute" field that you are referring to make each radio button go on its own line.
Can this not be done in version 1.6?
