Hi
This plugin seems to be working well, but we have one issue...
What we want, is for new subscribers to have to be approved before they can log in to their account. However, instead they are immediately able to log in.
In other words, new users should be listed as PENDING until we have approved them. However, they are immediately being made ACTIVE instead.
So, how can we ensure that all new subscribers, even through they have paid, stay as PENDING until we manually allow them to be ACTIVE?

First of all, I am sorry for lately response. I am on new year holiday at the moment and only come back to office on 6th, Feb.
Back to your questions, you can follow the instructions below to make it works like that:
1. Go to Extensions -> Plugins Manager, find and make sure you publish the "OS Membership User" plugin.
2. Go to Users -> Users manager, click on Options button in the toolbar, then set "New User Account Activation" to Admin.
After that, everything should work as expected.
