Sorry, I am new to this extension and I operate from the UK and the default address fields of State and Zip don't apply. In the UK they should be County and Postcode. I have just found the Translate tab which allows me to rename fields, which is great.
There is a small problem. The Country field will always appear between City and State fields although o the Fields tab it is listed between Zip and Phone, which is more logical.
So, would it be possible to allow users to alter the field order or at least move the Country field..
Also I note that although the other fields are there, the First Name field does not appear on the Field or Free download form field tabs for some reason, so it could be added for completeness.
By the way, I am really enjoying using DMS so far. The more I use it the better it gets.
