DMS 2Checkout Payment Gateway Issue

DMS 2Checkout Payment Gateway Issue was created by Don Viar
I have installed the payment gateway plugin per the instructions in the Readme.txt file, but I don't see a screen that let's me configure the actual settings for the payment gateway.
Under "Payment Plugins", when I click on the os_2checkout plugin, I can see an area on the right hand side of the screen called "plugins parameter" but it is blank. There are no visible fields to populate. (see attached screenshot).
I have tried to open the admin in both Chrome and Safari with the same results.
Please advise on what I am missing.
