Subject: Form format, title and payment problems

Hi, a few of questions. Firstly on my attached photo screenshotdocu.jpg the form elements seem to be very close together. How can I separate them. Give more space between the elements.

Secondly, On pic1.jpg of my attachment I wish to include a title such as " My online document shop" around the red circle area. There doesn't seem to be an option to configure that. Can you let me know how I can do it?

Thirdly, on the checkout page can I change the "paypal" option to "paypal and credit card"? because I know you can just type in your creditcard information on paypal. So changing the title seem more appropriate.
