Form Title

The words 'Payment for' appear before the title of my form. I want to change the wording but cannot see how to do that.

If I cannot do it from the Joomla backend, can you tell me the directory & file name where I can find the file so that I might easily change it myself.

Go to Payment Form -> Translation, you should be able to see the text and can change it to remove the word.
