Donation History Receipt Format

I have a couple of issues with my Donation History Receipt format. I'm trying to use an image tag at the top, that is being completely ignored. In the email receipt, I'm getting the alt text, but the pdf version seems to ignore the <img> tag altogether. A <span> tag to change the font size is also being ignored.

The [CAMPAIGN] and [DATE] variables are not being replaced in the body of the receipt.

Just a little "gotcha!" for those trying to edit their Donation History receipt. When you create the receipt for the first time, it stores a copy of the receipt in /media/com_jdonation/receipts, so if you use that receipt to test again, you get the stored copy, not a fresh copy. I kept making changes to the receipt layout and kept getting the same output. I won't mention how long it took me to find the copies, since they were exactly where they should be.

The <img> tag was bad html on my part, I was using "width=100%" inside the tag, should be style="width:100%;"
