Donation Form does not look right

We are upgrading a site from joomla 1.5 to joomla 3.0.
You can view the comporent live on the 3.0 site at: 198.20.227.139/~caninees/
The old site is at: canineestates.com/

On the home page in the right sidebar, if you say you are going to donate $10 - it takes you to the form page. But - the page does not look right even though the settings are identical to the old site. (see ScreenShot-New.jpg attached).

It does not take the $ into the form and there is not a submit button.
We want it to work like the old site (see screenShot-Old.jpg attached).

What do we need to do to fix this?

It seems the problem causing by a payment plugin. Could you please submit a support ticket sending us administrator account of your site so that I can check it for you ?
