Partial Page Displayed after Upgrading

I just upgraded to the new version, and the donation form no longer displays in my page template, plus only part of the donation form displays. See attached screenshot.

There must be something special on your site causing the issue. Please submit a support ticket sending us admin and FTP account of your site so that we can check to see what's wrong and get it corrected for you

I had created a ticket for this earlier but without the ftp and admin login information. I have just added that to the ticket. The ticket ID is 20333.
