Could you please go to Payment Form -> Configuration, find the config option "Load twitter bootstrap", set it to Yes and check it again?

The configuration is already set for Yes to Load twitter bootstrap.
I'm wondering if it's a css clash with either my template or another component I'm using RS Forms Pro ?

That's strange to me. Could you please submit a support ticket sending us super admin account of your site so that I can check to see what's causing the issue and get it sorted ?
