Form field question; style

Hi there. I have two questions and attached a screen shot.

1.) The height of the form field cuts off the "Other Amount" text .

2.) Under the "Amount" field, I have a $ symbol and the the text field starts underneath the $ symbol. How can I move up the field one row so that it is next to the $ symbol?

I think there are some css conflict on the site causing the issue. Could you please submit a support ticket sending us super admin account of your site and the link to the page so that we can check to see what's wrong and get it sorted for you?
